How care works here
Three things decide what an appointment at Stillmind is like: progress you can see on paper, prescribing that stays cautious and gets explained, and staying with the clinician you were matched with.
You keep the clinician you start with
You are matched with a clinician at your first appointment and you stay with them. You do not start the story again every few months, and the person prescribing in month nine is the person who watched months one through eight. That is the difference between a treatment history and a list of medications.
It matters most in the parts of this work that are slow. Whether a medication is helping is often a judgement about the last three months rather than the last three days, and that judgement is much better made by someone who was there for them.
Measurement-based care
Standard measures such as PHQ-9 and GAD-7 are repeated over time, so progress is something you can see on paper rather than something you try to recall across a gap of six weeks. Memory for mood is poor, including yours and including the clinician's.
When the numbers and how you feel disagree, that disagreement is interesting and gets discussed. The measures inform the decision and do not make it.
Conservative prescribing
One medication at a time, started low, changed on evidence. Every prescription comes with what it is meant to do, how long that takes, what to watch for and what the alternatives were. Declining a medication is a real option and the conversation continues from there.
Fewer medicines at effective doses is the goal, and coming off something is as much a part of the work as starting it. See medication management for the detail.
What this practice does not claim
Psychiatric treatment helps most people and it does not help everyone, and nobody can tell you in advance which you will be. First choices often turn out to be wrong ones, and finding the right treatment can take months. That is the ordinary course of this work and not a sign that it has failed.
Some conditions are managed for life rather than resolved. Where that is true of yours, you will be told, because a plan built on a promise nobody can keep falls apart at the first setback.
There are also things this practice does not offer: medically supervised detoxification, residential treatment, neuropsychological testing batteries, and specialised trauma protocols such as EMDR. When one of those is what you need, you will hear it directly and be pointed toward it.
